[Urgent] Related to BSNL security refund. Need Help

Hi, I am surrendering my BSNL landline|broadband tomorrow. We recently moved to a new house and the connection was on our previous address. So How to tell BSNL to send me security refund on my new address instead of my old address.
Can i just mention my new address in my application written for surrendering connection or i have to do some other step? Please help i am confused.

Last time i surrender my bsnl wimax and after that we moved to new house and the security cheque never came. It was 500rs so i did not bother but this time the security cheque would be up to 3000rs so i do not want lose it. Please tell me what i need to do.

I was thinking same but should not i mention the address where landline was installed. I do not want to confuse govt employee.

Mention your Bank/NEFT details :/ they will directly send it to your bank account, regardless of your address
