SBI PAI - Unauthorized Charge

I just checked my SBI bank account statement and found this:

I checked online and it seems to be some personal accidental insurance given by SBI, but the problem here is that I don't even use this bank account. And I don't remember signing up for it ever. Is this insurance mandatory or voluntary?
 
Voluntary.
Talk to your branch manager and get those charges reversed.
 
I tried to raise a complain online, and it simply blocked me as I hit send. And I got a text.

Last year they charged me some debit card annual fee, when I don't even have an active debit card for this account.
 
Same thing happened with my parents account.
Sbi gen accidental insurance .
They fool you to sign this when opening account. Sometimes do themself.
Was 100 per year till last year now suddenly hiked to 500 per year without consent.
Was not removed even on written complaint.
So i just withdraw all money and opened account in hdfc.

This account is more than 10 years old though. And this is the first time they've deducted this amount, so I really doubt I signed up for this while opening it. I mean it's fine if they have given me an insurance, but at least they can send a certificate for the same? For that, I've to visit the branch now. I mean.
