IPTV Settings Lost

Hey friends, I am in great problem. Here is the description, please help me out...

I was having BSNL IPTV installed on my modem, and today it accidentally got reset. Please tell me how to configure my ports in my modem(DNA-A211-1, 4 outputs). If anyone is having iptv installed, please share your modem's setting's configuration file, so that I can port map my modem for iptv.
 
is this IOL iptv?
 
Why dont u call customer care? Since this is basic problem they shud be able to solve it on phone itself (even if guy on phone knows nothing he will simply read the instructions in front of screen)
 
Can you post the IPTV settings which you recall to set that service as i have currently lost them due to restoration of my router to factory settings.

do you need to do this for all interfaces? cause IOL required only 0/100 to be set to bridged!!
 
I've only used nas_0_100 interface, you can notice, the group name "iptv" I've created...
