I am guessing this is the certificate for comorbidities that is required. Correct me if I am wrong.

It's the same form which was required to be filled by individuals above 60 years of age or with comorbidities. I think it was supposed to be attested by a doctor.

It must also be noted that children registering for the Covid-19 vaccination will only be jabbed with the indigenous COvid-19 vaccine Covaxin developed by Bharat Biotech.
