Is anybody else getting the message at the login page:

As per the directives of Government Authority, customers are requested to fill up the Customer Agreement Form along with a photo ID proof and residential address proof within 30th September 2014. If not complied then it will be notified to the Authority. ​

Does anybody have any information regarding this??
