Group error displayed always

hi,
am new to this forum..
hope to find out the solution to my problem.
i use Netgear adsl modem dg632 and i get this error always when i connect.
"00:06:16 Got group error |Bad username/password|
00:17:10 Got group error |Bad username/password| "
kindly help me.

i couldnt find the detaikls in my modem
i use Netgear and it shows only the following
