My Internet goes down for 5 min. every 2-3 hours

This is the error log from the modem.

Also the internet light on modem goes red and then at the end the DSL and Internet lights go off and come back again with everything working fine again. The browser shows no connection or unable to reach dns server sometimes. Any solutions would be appreciated too.
