i believe you can turn on peer to peer for all for voice and video calls on telegram.

So, individual channels can be blocked.
 
Telegram can of course block channels and groups and individual accounts 🤷🏼

Telegram would be forced to block them on ios to keep the app listed on app store. I don't see how Apple can block in app content.

But yea... Usually proxy or VPN doesn't help when this happens. Likely based on where affected accounts are based in.
