How to get a duplicate SIM Card for BSNL Mobile Connection?

I lost my bsnl 3g sim after I kept it in my purse. Whats is the procedure to get duplicate sim? Getting conflicting answers in the internet. How much should I have to pay and should an FIR be necessary for getting duplicate sim?
 
Go to police station File an FIR. then, Take your id proof and photograph with you to the exchange. They will charge you 150/-
 
No need of FIR or police intervention. If the applicant name, photo and address matches the lost or damaged sim records, The same person has to be in person to get the sim.
 
From what I remember, I was able to get a nano-sim for my number without giving my old sim to the idea store. So you should not require a FIR if you just say that you bought a new phone and it does not support your current SIM 😛
 
no FIR needed just head to exchange get a new sim , and your old will get deactivated.
 
For companies like airtel and docomo, no need for fir and all that stuff. But this being bsnl, im apprehensive. I hate going to bsnl exchange 😛
 
Update: Went to telephone exchange. Internet was slow(the irony) but once they saw my details, they gave me form to fill up. Took that form, address proof and passport photo & 20rs and gave me new sim. Told me it'll be activated in 2 hrs and my balance in old sim will be carried forward. Got out in 20 mins. Good stuff.
 
@dextermorgan Did you had to visit the telephone exchange of the same city from where sim was issued? Once my cousin needed to replace sim in Varanasi which was issued from Allahabad and the exchange guys asked him to visit Allahabad, reason given was that records of that sim were not available at that exchage.
