Is my username hacked?

Hi, I think my old modem which was returned to shop, I forgot to reset modem to remove username & password. :/
I'm not referring to modem's 192.168.1.1 login.
but, The one on the bill which includes your initial, some no.s from land-line (phone no) ending with scdrid.
Is it possible for someone to access internet using my credentials? of course from a different line.
I can't access the net for whole daytime-night 7:00 A.M - 11 P.M
Both the LAN & DSL light on modem doesn't come at all.
I'm assuming connection is used by someone at that time.
& now too, very unstable connection. 5-10kBps
I haven't got the monthly starting speed, 200-250kBps & I'm not getting the minimum speed 50-60kBps I should be getting for a whole month, either.
It's been 4-5 days since the payment is done.
How could I reset/ change username & password?

i am pretty sure bsnl has port binding.
 
Not possible. Now a days, BSNL binds your connection to your phone no. hence making it impossible for others to use it.
